General Description
The Speech Therapist is responsible for the initial screening, annual screening, and screening as needed of individuals served to identify individuals needing further evaluation. He/she is responsible for developing care plans for residents. He/she must be well versed in dysphagia and have the skills necessary to perform bedside evaluations and make appropriate recommendations for the intellectually/developmentally disabled. He/she must be familiar with International Dysphagia Diet Standardization Initiative (IDDSI) and have one year of dysphagia experience. He/she acts as a positive role model for staff and individuals residing and working at Mount Aloysius. He/she must have a working knowledge of ICF/IID standards, Licensure regulations, and facility policies and procedures. The position will exert 10 to 25 pounds of force frequently to move objects. In accordance with the U.S. Department of Labor physical demand strength ratings, this is considered light work.

Specific Responsibilities
1. Initial screening of residents to identify individuals needing further evaluation and treatment
upon the request of Qualified Intellectual Disability Professional, Program Director, Medical
Services, or Administrator/DOO.
2. Provide speech therapy evaluations for residents annually and as needed and the necessary
written reports to document evaluation results with recommendations for treatments.
3. Identify resident goals for treatment.
4. Appropriate speech therapy services will be provided in accordance with each resident’s plan
of treatment.
5. Maintain complete and accurate records of all care, treatment, and examination.
6. Consultation with staff on treatment plans and progress.
7. Provide training to facility staff as needed/requested.
8. Perform other duties as assigned by the Administrator/DOO.

Skills and Abilities Required

  • Master’s Degree in Speech Language Pathology.
  • 1-year experience with dysphagia.
  • 1-year experience with IDDSI.
  • Ability to organize and prioritize work, and complete tasks in a timely manner.
  • Possess good verbal and written skills.
  • Ability to analyze information and produce clear, concise reports and correspondence.
  • Ability to work in a professional manner.
  • Ability to keep all information confidential.

Assistant Program Director

Mount Aloysius, an Intermediate Care Facility for Individuals with Intellectual and Developmental Disabilities, is currently seeking an Assistant Program Director to join our team.  Mount Aloysius provides residential and day services for people with developmental disabilities. The great men and women who work for Mount Aloysius are focused on helping the people they serve live healthy and quality lives. The Assistant Program Director plays a key role in what we do. The Assistant Program Director is responsible for monitoring and directly coordinating Shift Supervisors and indirectly coordinating all Program Services and Direct Support Professional Staff.  The Assistant Program Director monitors staff to ensure resident and facility requirements are met as evidenced by completed documentation.  He/she acts as a positive role model for shift supervisors, staff and individuals residing and working at Mount Aloysius.  He/she must have a working knowledge of ICF/IID standards, Licensure regulations, and facility policies and procedures. 

 

Duties include but are not limited to:

  • Schedule Shift Supervisors and arrange for annual leave and for call off days.
  • Act as a liaison between Shift Supervisor and Program Supervisor as well as Program Director.
  • Direct supervision, communication, coaching and training of Shift Supervisors and DSP staff, including managing performance and administering progressive discipline.
  • Develops and maintains confident and competent staff who deliver continuous improvement results.
  • Assures that all assigned staff have the training, tools, and equipment to complete and perform their responsibilities.
  • Coordinates with all Shift Supervisors to provide seamless shift turnover and manpower coverage.
  • Enforces continuous active treatment by conducting multiple quality assurance checks per shift.
  • Facilitates the recognition of all team members who achieve positive results to instill appreciation of the value of quality care.
  • Supervise and coach staff to ensure that standards of cleanliness are maintained.
  • Responsible to communicate information/directions to Shift Supervisor.
  • Chairperson for Shift Supervisor meetings.
  • Ability to assume Shift Supervisor duties when an absence occurs.
  • Supervise Shift Supervisors ensuring that facility policies and procedures as well as the contract between Mount Aloysius and UMWA are followed.
  • Maintain flexible hours to monitor all three shifts promoting teamwork, problem solving, and good communication.
  • Complete Shift Supervisor and Direct Care Professionals evaluations.
  • Responsible to schedule and coordinate the master schedule to ensure apartment coverage.
  • Receive telephone calls concerning staff call-offs; document call-offs on appropriate form and notify appropriate personnel of call-off.
  • Call in substitute staff; document and maintain a record of call-ins on appropriate forms.
  • Assist in crisis situations and ensure proper documentation is completed as required.
  • Assist in fire drills/emergency actions.
  • Maintain a log of shift activities in Shift Notes.
  • Assist in monitoring resident meal time; ensuring adequate staff and appropriate staff behavior.
  • Coordinate search for missing resident(s) according to facility policies and procedures.
  • Provide housekeeping supplies to the apartments as needed.
  • Route maintenance Work Order Requests.
  • Assist nursing staff in coordination of medical/dental appointments, schedules staff, and notifies appropriate personnel of the appointments.
  • Oversee that staff arrive to work on time, promptly go to the assigned apartments, and do not abuse breaks or meal periods.
  • Issue disciplinary action as needed.
  • Distribute documentation forms to staff, collect completed forms as requested, and distribute completed forms to proper personnel.
  • Assist in the interview process and provides on the job training, acting as the point person for all new Direct Support Professionals.
  • Coordinates monthly Shift Supervisor turn over and regular resident inventory.
  • Conduct regular check ins with new staff to ensure competency and the need for additional training.
  • Participate in continuing in-service/educational training in order to strengthen and increase knowledge and skills necessary to meet the needs of the residents and facility.
  • Lock exterior doors and provides security checks periodically for all buildings in the immediate area.
  • Assist in orientation and instructions of new personnel as well as training for new or less experienced Shift Supervisors.

  • Bachelor’s degree preferred
  • 1-year experience with DD population/ Supervisory position preferred.
  • Maintains a valid driver’s license to operate a company vehicle.

Medical Assistant

We are currently seeking Medical Assistants to join our team.  The Medical Assistant plays a key role in what we do. The Medical Assistant provides individualized nursing care to each resident based upon the residents’ specific needs and follows all applicable regulations to ensure confidentiality of resident records.

Your Responsibilities Will Include:

Carry out administrative and resident care duties as directed.

Perform administrative duties, give and receive reports, answer incoming telephone calls.

Order prescribed medications and transcribe physician orders to resident Medex.

Assemble resident chart, ensuring all permits, authorizations, releases, and other required forms are attached.

Requisition supplies, equipment, linens, and emergency drugs to meet department needs. Ensure equipment is in good operating order.

Maintain department records, performing charting duties as indicated and complete required department or medical chart forms.

Prepare residents and transport residents to appointments for any services outside the facility; notify appropriate personnel if transportation is to be arranged.

Make daily visits to observe and evaluate residents’ physical and emotional status; monitor vital signs, weight monthly, and record.

Keep the Nursing Supervisor informed of the status of residents and other related matters through written and verbal communication.

Administer medication with the exception of controlled substances (narcotics), give treatments, and perform other professional services as ordered. Provide emergency treatment when required; notify Physician of emergency, take and carry out Physician’s orders.

Assist the Physician with examinations and treatment, notify the Physician of changes in condition and any unusual or abnormal behavior.

Obtain and maintain First Aid/CPR/AED Instructor Certification, train new employees in First Aid/CPR/AED and recertify current employee certification as needed.

Train new employees, existing employees who may need retrained, and annually inservice all employees on Blood Borne Pathogens including universal precautions for infection control including hand washing and the disposal of bodily waste.
